How to Track IEP Goals Digitally at Your Therapy Centre

Write each IEP goal as a measurable target, log progress against it every session from a phone, and keep one shared goal sheet per child. Trends, progress reports and therapist handovers then come from the same record.

Most centres still track IEP (Individualised Education Programme) goals on paper or scattered spreadsheets.

    Why Paper IEP Tracking Fails

    • Notes get lost: Paper files are misplaced, damaged or illegible, and often leave with the therapist.
    • Progress is invisible: You cannot see whether a child is progressing, plateauing or regressing on a specific goal.
    • Reports take hours: Quarterly reports for parents or schools mean searching months of paper notes.
    • Handovers break continuity: A new therapist cannot see which goals were worked on or where progress stands.

What Digital IEP Tracking Looks Like

    1. Structured goals: e.g., "Produce /r/ in initial position with 80% accuracy across 3 consecutive sessions"
    2. Session-by-session logging: goals worked on, performance and observations
    3. Trends: improvement, plateau or regression per goal
    4. Automatic reports: goal progress compiled for parents or schools
    5. Instant handovers: a new therapist sees all goals, status and history

    Examples Across Therapy Types

    Speech therapy IEP goals:

    • Produce target phonemes in isolation/words/sentences with X% accuracy
    • Follow 2-step verbal instructions in 4 out of 5 trials
    • Use 3-word phrases spontaneously in 80% of observed interactions

    Occupational therapy IEP goals:

    • Maintain seated posture for 15 minutes during tabletop activities
    • Complete a 10-piece puzzle independently within 5 minutes
    • Tolerate tactile input (textured materials) for 30 seconds without aversion

    ABA therapy goals:

    • Mand (request) for preferred items using picture exchange in 80% of opportunities
    • Reduce self-stimulatory behaviour to fewer than 5 instances per 30-minute observation
    • Follow group instructions with no more than 1 prompt across 3 consecutive sessions

How to Transition from Paper to Digital

    1. Start with new patients: Track all new assessments and goals digitally; do not digitise years of paper records.
    2. Enter current goals for existing patients: Enter active patients' current IEP goals. Historical notes can stay on paper.
    3. Train therapists on mobile note-taking: Notes take 2-3 minutes on a phone, not 10 on paper.
    4. Set a cut-off date: After it, all session notes and goal tracking are digital, with no paper backup.

    Goal Tracking Across Multiple Therapists

Keep one shared goal sheet per child when more than one therapist works with them: a regular SLP plus a substitute, or speech therapy and OT at the same centre. On paper, each therapist keeps separate notes and the goal picture fragments.

    • Every therapist logs against the same goals: phoneme targets, motor milestones or behaviour goals, so trends stay continuous.
    • Substitutes see the full history: a covering therapist reads recent notes and goal status before the child walks in.
    • Cross-discipline goals stay visible: speech and OT therapists see each other's goals, and reports combine both.

In a multi-therapist centre, this is what keeps goals from resetting when a therapist leaves. See it in our speech therapy software and occupational therapy software.

    How Theraflow tracks IEP goals

Therapists set IEP goals, log progress each session from their phone and generate progress reports automatically. Parents see milestones in the parent app, and handovers take 60 seconds.
